INFORMED CONSENT
You are invited to participate in a research study that will involve participation in an online survey focusing on intergenerational communication. This research is being conducted by Rachel Stark, Health Sciences Librarian at California State University, Sacramento (CSUS) and Jenessa McElfresh, Health Sciences Librarian at Clemson University. You were selected as a possible participant in this study because of your participation in this list-serv.
The purpose of this research is to survey your attitudes, beliefs and perceptions of intergenerational communication among Health Sciences and Medical Librarians and Information Professionals. Your participation in this survey will last approximately 10 minutes.
Risks attributed to the study are not anticipated to be greater than those risks you encounter in daily life. We cannot guarantee that you will personally experience benefits from participating in the study; however, others in the future may benefit from information we find as a result of this study.
To ensure your confidentiality, responses to this survey will be collected in an anonymous fashion. The data obtained will be maintained in a safe, locked location and will be destroyed after a period of three years after the study is completed.
Your participation is entirely voluntary and your decision whether to participate will involve no penalty or loss of benefits to which you are otherwise entitled. If you decide to participate, you are free to discontinue participation at any time without penalty or loss of benefits to which you are otherwise entitled.
